Well, you've already done the hardest thing, which is to start! The rest is easy as long as you don't stop :)
Sounds like you have a good mix of cardio and strength training already. If you want a specific outline, here's mine:
Day 1:
- cardio: bootcamp
- strength: arms (bi's & tri's)
Day 2:
- cardio: 10K row
- strength: back
Day 3:
- cardio: 7 mile run
- strength: shoulders
Day 4:
- cardio: kickboxing
- strength: legs
Day 5:
- cardio: indoor sprint triathlon (run, row, bike)
- strength: chest
Day 6:
rest :)
Day 7:
- cardio: endurance run (normally 7-10 miles, but more now since I'm training for a marathon)
- strength: bodyweight (pushups, pullups, dips, squats, lunges)
The actual exercises aren't set in stone because I like to mix things up to stay motivated. For example, "shoulders" day isn't always the same thing - one week I'll do max resistance, low reps and the next week I'll do lighter weight, max reps.
Also, I may switch the cardio/strength combinations from time to time to keep things from becoming too routine.

I would wait a full 3 days after all the symptoms are gone. Although you are feeling better, your immune system is not at 100%, and no point in getting sick all over again. When you do return, make sure to increase the amount of anti-oxidants, L-Glutamine and Vitamin C to insure your immune system stays working at prime.
